Update to Online Provider Onboarding Process for Joining Our Networks
Effective January 1, 2019, Blue Cross and Blue Shield of Texas (BCBSTX) will only be accepting the new Provider Onboarding Form electronically 
. The previous paper forms will no longer be accepted. The change will streamline the application process and lessen the amount of time it takes to get a response.
The Provider Onboarding Form is used by BCBSTX to set up a provider record number and send contracts for network participation. Providers can locate the new electronic Provider Onboarding Form on the provider website under Network Participation/ How to Join / Provider Onboarding Request.
The form should be filled out by:
- Individual providers new to our networks
- Groups and clinics new to our networks
- Individual or Group/Clinic providers who already have a Provider Record ID and want to join our networks now or add any additional network contracts
- Existing contracted groups or clinics who are adding a new provider
Note: Facility/Hospital Based providers, be sure to include the name of each Hospital or Ambulatory Surgery Center that the provider services in the Comments section of the Provider Onboarding Form when adding new individual providers or in the comments field of the Provider Roster template when adding providers to an existing contracted group/clinic.
