An update was made to the Urine Drug Testing Documentation Guidelines 
posted to the Blue Cross and Blue Shield of Texas’ (BCBSTX) provider website under Claims and Eligibility/Claims Filing Tips. It is intended to assist providers in understanding BCBSTX documentation and medical necessity requirements for urine drug testing claims.
Refer to this document as well as the BCBSTX Medical Policy MED207.154, “Drug Testing in Pain Management and Substance Use Disorder Monitoring”, for information related to Urine Drug Testing documentation.
